Brief Questionnaire about Extended Access Appointments

Our Primary Care Network becomes responsible for access to General Practice during extended hours in October. We have sent a text to as many of our patients as we can to ask preferences for this (5 questions). Responses will be collated with those of other practice’s patients to inform planning, to try and meet as many people’s needs as possible.
